Any service provided by Senior Care Home Health (hereinafter called Senior Care) is subjected to this legal agreement between you, the subscriber, and Senior Care. Unless otherwise agreed in writing by Senior Care, your agreement with Senior Care will always include at a minimum the terms and conditions set out in this document. In order to use any Senior Care service you must first agree to the terms. You may not use the services if you do not accept the following terms:

• Senior Care is responsible for providing the health care services as stipulated in the enrolled plan only to the beneficiary designated in the enrollment application.
• It is a precondition that the service provided by Senior Care are only to the extent mentioned in the Plan in the Senior Care Website and will not extend to any other aspects or further period unless and until agreed/extended in writing by Senior Care.
• Senior Care is not responsible for any pre-conditions or any other illness or diseases that the beneficiaries might have contracted during the period
• Senior Care does not provide emergency care services, and will not be held responsible for emergency medical or surgical treatment.
• All services will be offered to my nominated beneficiary by Senior Care at their convenience at the home of the nominated beneficiary only.
• The nominated beneficiary cannot be changed during a subscription period.
• My beneficiary shall abide by the rules and regulations, which Senior Care makes from time to time.
• Senior Care may alter the terms and conditions of this Contract with a notice period of 30 days.
• Senior Care may charge me for any additional expenses at actuals in which case I will be notified before hand.
• Senior Care services are administered for a period of one year and for one year increments there after and there is an option to pay the membership fee on an yearly basis or as 12 monthly installments. Any premature termination within the one year term period will result in a service cancellation fee equivalent to the cost of 3 months of service
• Payments are due at the time of enrollment and once every 30 days thereafter for monthly installment payment option
• A late fee of $25 will be charged if payment is not received by the due dates
• If payment is not received within 15 days of due date, the services to the beneficiary will be suspended. All scheduled visits and services during the suspended period will be cancelled and will not be rescheduled. Access to the member area of the website will be restricted at that time. The account may be reactivated by paying a reactivation fee of $50 in addition to all defaulted payments
• I shall indemnify Senior Care for all the losses, if any sustained to Senior Care on account of this Contract.
• Senior Care shall not be liable towards any payment in respect of any claim, if such claim be in any manner intentionally, recklessly or otherwise misrepresented or concealed or non disclosure of material facts or making false statements whether by me/us on behalf of the beneficiary and such action shall render the Plan accepted by me/us as null and void and all claims if any by me/us shall be forfeited.
• Senior Care can take suitable legal action against me/us as per Law.
• Senior Care reserves the right to provide service to anyone and may without assigning any reason cancel the plan by sending me 30 days notice. I agree that Senior Care may provide me with notices, including those regarding changes of the terms, by email, regular mail, or postings on the services.
• Senior Care is not responsible for inability to provide services as a result of an external factor such as but not limited to severe weather conditions or natural calamities or local political situations.
• Senior Care legal contesting will fall under Trivandrum jurisdiction.
• If any court of law, having the jurisdiction to decide on this matter, rules that any provision of these terms is invalid, then that provision will be removed from the terms without affecting the rest of the terms. The remaining provisions of the terms will continue to be valid and enforceable.
• By signing up for Senior Care Services I declare that I have received permission from my beneficiary to enroll him/her for Senior Care services.
• By signing up for Senior Care services I agree that I shall abide by Senior Care Terms and Conditions vested here. Only Senior Care may alter the terms and conditions of this Contract.

Password and Accounts:

• I agree and understand that I am responsible for maintaining the confidentiality of passwords associated with any account I use to access the services.
• Accordingly I agree that I will be solely responsible to Senior Care for all activities that occur under my account.
• If I become aware of any unauthorized use of my password or my account, I agree to notify Senior Care immediately at customercare@seniorcareindia.com
